29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Lower Roof Deck - Employees were exposed to the hazard of falling approximately 16 feet from the roof edge to grade while required to dispose of roofing materials and debris where fall protection had not been provided. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.502(j)(7)(i): Materials and equipment were stored within 6 feet (1.8 m) of a roof edge where guardrails were not erected at the edge: a) Upper Roof Deck - Employees were exposed to the hazard of falling approximately 23 feet from the north edge of the roof deck to grade where roofing materials were stored within 6 feet of the roof edge without fall protection. b) Upper Roof Deck - Employees were exposed to the hazard of falling approximately 7 feet from the west edge of the roof deck to a lower roof deck where a generator was stored within 6 feet of the roof edge without fall protection. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Lower Roof Deck - Employees were exposed to the hazard of falling approximately 16 feet from the roof edge to grade while engaged in roofing activities without the use of fall protection or safety monitoring. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection: a) Employees were provided with a Honda 6500 watt generator without having been provided with GFCI protection for cord and plug tools and equipment. No abatement certification or documentation required for this violation.
