Allen Logistics, Inc. located at 6245 Industrial Pkwy Whitehouse, OH 43571; On or about February 28, 2012, the employer did not assure the forklifts, used to transport and store pallets, were equipped with backrests. Employees are required to load and unload trucks, as well as transfer the pallets from the washer to storage and are exposed to struck by hazards from falling wet pallets.
