29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  On or about 01/31/12 at the construction site where employees were cleaning the front of the residential home from a welded frame scaffold 4 tiers in height and the work platforms were not fully planked, exposing employees to the hazard of an 18 foot fall.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that  each platform on all working levels of scaffolds will be fully planked or decked between the frontuprights and the guardrail supports.

29 CFR 1926.451(b)(5)(i):     The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilivered end.  29 CFR 1926.451(b)(5)(i):     On or about 02/08/12 at the construction site where employees were working from a welded frame scaffold which was provided with a scaffold plank 10 feet in length and which was extended 24 inches past the scaffold support frame, exposing employees to the hazard of an 18 foot fall.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that  each end of a platform 10 feet or less in length does not extend over its support more than 12inches (30 cm) unless the platform is designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, stucture, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about 02/07/12 at the construction site where employees were cleaning the stone vanier from a welded frame scaffold which was not provided with a ladder and employees were climbing from one level to another by using the crossbracing, exposing employees to the hazard of an 18 foot fall.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that  when scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portableladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps,walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface will be used. 29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about 01/31/12 at the construction site where employees were working from a welded frame scaffold 4 tiers in height and the scaffold platform was not provided with guard railing, exposing employees to the hazard of a 24 foot fall.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that  each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4)of this section.

Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, will be protected by protective helmets.
