29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: (1) Employees operating the Milltronics VM30 CNC Mill were not provided with locks and tags to attach to the energy isolating devices, prior to climbing into the CNC mill to clean the trough or prior to cleaning the clamps from the operator's door. (2) Not all the mechanical power press operators were provided with locks and tags to attach to the energy isolating devices, prior to wiping the dies while they were in the presses and wiping the bolster tables. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: (a) The periodic inspection of the energy control procedures did not include a review between the inspector and each authorized employee, of that employees responsibilities under the energy control program, including observing the authorized employees actually performing servicing and/or maintenance activities on machines or equipment, such as, but not limited to, the mechanical power presses, the laser machines, and the CNC mills. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: (a) On or about February 28, 2012, a press operator changed the dies on the Niagara 150 Ton Parial Revolution Press, without adequate training on the energy control program, including isolating and locking out the electrical disconnect and the pneumatic disconnect, as well as blocking the gravitational energy of the ram. (b) Press operators cleaned the dies and bolster table on the mechanincal power presses without adequate training on the energy control program, including isolating and locking out the electrical disconnect and the pneumatic disconnect, as well as blocking the gravitational energy of the ram. (c) Milltronics VM30 operators set up, cleaned and replaced tooling and other parts on the machine without adequate training on the energy control program, including isolating and locking out the electrical disconnect and the compressed air. (d) TruLaser 5030 operators set up, cleaned and replaced tooling and other parts on the machine without adequate trianing on the energy control program, inlcuding isolating and locking out the electrical disconnect and the compressed gases. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.: (a) The guards on the Milltronics CNC Mill were not interlocked with the machine control system. (b) The guards on the TruLaser 5030 were not interlocked with the machine control system.
