29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:      (a) At the project site on or about February 24, 2012, and at times prior thereto, an energized extension cord was used to provide power to a DeWALT Drill (Model DW130V, Serial Number 735883).  The black casing that surrounds the connection between the "male" end and the outer sheath of the cord was pulled out, thus straining the cord and potentially causing the strands of one conductor to loosen from under terminal screws and touch another conductor.
