29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around each hole: (a) Rose Homes, Inc., @ 499 White Cloud, Breckenridge, CO 80424: On and before 2/24/12, the creating/correcting employer did not ensure that the basement access was adequately guarded. Employees utilized a ladder located inside the unfinished stairwell to access the basement. This condition exposed employees to a fall hazard of approximately 12ft. (b) Rose Homes, Inc., @ 499 White Cloud, Breckenridge, CO 80424: On and before 2/17/12, the creating/correcting employer did not ensure that the elevator shaft hole was guarded to prevent accidental exposure to a fall hazard of approximately 12ft.
