Employees utilized a ladder located inside the unfinished stairwell to access the basement. This condition exposed employees to a fall hazard of approximately 12ft.     (b) Rose Homes, Inc., @ 499 White Cloud, Breckenridge, CO 80424: On and before 2/17/12, the creating/correcting employer did not ensure that the elevator shaft hole was guarded to prevent accidental exposure to a fall hazard of approximately 12ft.
