29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:      A)  42 Hedden Terrace, North Arlington - NJ: Employees were exposed to fall hazards hazards of approximately 26 feet while using an extension ladder to access a scaffold platform. The extension ladder that was used, did not extend at least 3 feet above the landing, on or about January 25th 2012.

29 CFR 1926.1053(b)(4):     Ladder(s) were used for purposes other than the purpose for which they were designed:      A)  42 Hedden Terrace, North Arlington - NJ: An employee was exposed to fall hazards hazards of approximately 30 feet while using a folding step ladder in a semi open position, on top of a 26 foot high scaffold and using the top step of the step ladder. The step ladder that was used was not used according to manufacturers instruction, on or about January 25th 2012.
