OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to fire and explosion hazards: On or about February 23rd, 2012 employees were exposed to burn and explosion hazards due to LP gas accumulating inside a slot furnace when using an unattended hand torch as a heater in an area with inadequate ventilation. Abatement Certification and Abatement Documentation Required.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or group: On and at times prior to February 23rd, 2012 each maintenance worker that worked inside the 202 slot furnace did not lockout or tagout the energy isolating devices to provide them with the equivalent protection of personal lockout tagout procedures. Abatement Certification and Abatement Documentation Required.
