29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    A worker at the jobsite located at 3507 Hawthorne Boulevard, St. Louis, MO, 63104, was not wearing a protective helmet (hard hat) in an area where there was possibility of head injury.  The worker was observed performing removal of tear-off debris from the ground directly below other workers actively performing roofing activities on the roof approximately 10'3" above.

29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    Workers were using nail guns to nail shingles and performing roofing activities on the roof without safety glasses at the job site located at 3507 Hawthorne Boulevard, St. Louis, MO, 63104.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    At the job site located at 3507 Hawthorne Blvd, two employees were exposed to a fall hazard as they were not protected by any type of fall protection system while they actively performed roofing work on the 6:12 pitch residential roof.  The eave to ground height measured approximately 10 feet, 3 inches.
