29 CFR 1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level   landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m)   above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a   rigid support, and/or a grasping device, such as a grabrail, was not provided to assist the employees in   mounting and dismounting the ladder:    a) On or about February 8, 2012 - job site at 27-11 Bell  Blvd., Bayside NY 11360    Employees used a ladder to access the roof that extended 6 above surface being accessed.     WRITTEN ABATEMENT CERTIFICATION IS NOT REQUIRED.
