29 CFR 1910.212(a)(1):     One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    Zikainim, Inc. dba All American Nursing Home did not ensure that one or more method of guarding was provided to protect employees from rotating parts.    a) The Berkel mixer in the kitchen did not have a barrier or guard to protect the employees operating it from rotating parts.

29 CFR 1904.32(a)(4):     The employer did not post an OSHA 300A Form or equivalent by February 1 thru April 30.     Zikainim, Inc. dba All American Nursing Home did not post the OSHA 300A Form, a summary of work related injury and illness for 2011 on February 1, 2012.     a) At the time of the inspection on February 29, 2012, the OSHA 300A Form for calendar year 2011 was not posted. The OSHA 300A has to be posted from February 1 through April 30.
