29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    Entire facility:  the employer did not establish a program for the control of hazardous energy (lockout/tagout) for maintenance employee(s) performing servicing or maintenance on machines and equipment such as but not limited to injection molding machines, grinders and granulators.  Program requirements include:    1) Identification of hazardous energy sources   2) Documentation of machine-specific energy control procedures  2) Employee training and certification (documentation) of training  3) Periiodic inspections and their certification (must be conducted at least annually)    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.335(a)(1)(i):  Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    Maintenance employee(s) who performed live voltage measurements (240v to 480v) on equipment during servicing and repair did not wear appropriate electrical protective equipment, including but not limited to the following, to protect against electrical shock and arc flash/blast injuries:    a)     Rubber insulating gloves with leather protectors.  b)     Arc-Rated clothing such as arc-rated long-sleeve shirt and pants or arc-rated coverall  c)     Hearing protection (canal inserts)  d)     Arc-rated flash suit hood or balaclava    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c). 29 CFR 1910.303(b)(2):  Installation and use. Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling.    a.  #14 Vertical Press.  A relocatable power tap (electrical outlet strip) was used improperly in an industrial setting to provide power for lights near the vertical press in the production area.      b.  #10 Vertical Press.  An electrical pendant box with multiple outlets (110v) was used on the end of a flexible cord to provide a power source for a light near the #10 vertical press.    There is no abatement documentation or certification required for this item.
