29 CFR 1910.119(d)(3)(ii): The employer failed to ensure that equipment used the process system complies with recognized and generally accepted good engineering practices. a) Dominicks Finer Foods of Illinois - Located on the roof of the west engine room: The ammonia relief system header discharge piping along the north wal, was located approximately less than 10 feet above the roof surface. b) Dominicks Finer Foods of Illinois - Located on the roof of the west engine room: The ammonia relief system header discharge piping along the east wall was located approximately less than 10 feet above the roof surface.

29 CFR 1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process: a) Dominicks Finer Foods of Illinois - The employer's process hazard analyses did not address hazards of its ammonia refrigeration processes. Hazards not addressed include: 1) The absence of visible and audible ammonia alarms inside the engine rooms, and outside each entrance to the engine rooms, with manual resets located inside the engine rooms and with the meaning of each alarm clearly marked by signage near the annunciators. 2) Emergency shutdown procedures including precautions to be observed in case of a breakdown or leak, were not posted outside each engine room, immediately adjacent to each door. 3) The absence of legible permanent signs, securely attached and easily accessible, reading "Engine Room - Authorized Personnel Only" and further communicating that entry is forbidden except by those personnel trained in emergency procedures when the ammonia alarm has been activated. The signs were not located on the following doors: The Viter engine room east door that communicate(s) with the electrical utility room. The Vilter engine room south door that communicate(s) with the facility near the employees break room. The Produce engine room west door that communicates with the warehouse of the facility.

29 CFR 1910.119(e)(5): The employer did not establish a system to assure that the process hazard analysis team's recommendations are resolved in a timely manner and that the resolution is documented: a) Dominicks Finer Foods of Illinois - The employer did not establish a system to assure that the process hazard analysis team's recommendations for the 1996 Process Hazard Analysis and 2011 Process Hazard Analysis Revalidation. The employer did not resolve and document in a timely manner its process hazard analyses (PHA) teams safety recommendations made in the May, 1996 (PHA) as follows: PHA (1996): Compressors: Test cutouts annually. Clean Strainers annually. Conduct routine PM inspections. Evaluate system conditions daily including pressures. Test control circuits and floats (vessels) annually. Monitor oil temperatures continually and verify cutout set points during PM. Download and check setpoints according to PM schedule. Receivers: Routine inspections. Routine PM on pressure regulators. King solenoid valve is inspected routinely. Ammonia Liquid Pumps: Routine PM on pumps and makeup solenoids. Check alarm periodically and liquid levels daily. Routine PM on flanges and valves. Operator training lockout/tagout. Routine inspections. Check/test float operation and alarms according to PM schedule. Check makeup solenoids and strainers periodically. Liquid Transfer System: Check float operation and operation of 3-way valve and check valves. Check transfer system operations according to PM schedule. Routine PM inspections. Oil Pots: Operator training. Check alarms according to PM schedules. Daily inspections. Ammonia Delivery: Operator training and charging procedure action plan. Review safety procedure with delivery vendor. Evaporators: Follow PM program for associated controls and units (including solenoid valves and strainers). Review system status printouts daily. Glycol Chillers: Follow PM schedule for system controls. Periodically inspect chiller tubes approximately every 7 years. Check detector calibrations according to PM schedules. Air Purger: Perform PM purger and controls per manufacturer's recommendations Condensers: Test high pressure cutouts at least annually. Fan performance is monitored continually. Tubes inspected as part of PM program (includes water treatment program). Piping: Inspect routinely for improper storage. Replace questionable sections of pipe. The employer did not resolve and document in a timely manner its process hazard analyses (PHA) teams safety recommendations made in the May, 2011 (PHA) as follows: AUB2012-02 - Ensure the P&ID are complete and up-to-date AUB2012-04 - Complete an evaluation of the contractor installing the control system once the project is complete. AUD2012-06 - Add inspection of evaporators piping etc. to daily or weekly inspection sheets.
