29 CFR 1926.403(b)(1):     Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations:    On or about February 24, 2012 on the north side of the building, employees were exposed to electrical shock hazards from exposed conductors on a brick saw.

29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.    On or about February 24, 2012 at the front entryway, employees were exposed to fall hazards when scaffolding legs were not supported by mud sills.

29 CFR 1926.451(e)(9)(iv):     Cross braces on tubular welded frame scaffolds shall not be used as a means of access or egress.    On or about February 24, 2012 at the main entryway, employees were exposed to fall from heights hazards when workers were using the crossbraces to gain access to the scaffold.
