29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable: a) A written Respiratory Protection Program which includes a respirator medical evaluation and/or fit testing was not established and implemented for employees who were required to wear 3M 1/2 face tight-fitting respirators in the Metal Pouring Area and a Clemco Supplied Air Respirator in the Blasting Area.

Each employer who has a workplace or work operation covered by this standard shall determine if any employee may be exposed to lead at or above the action level. a) The employer failed to provide initial montoring for employees working in the Blasting Area who were exposed to an 8-hour TWA (Time Weighed Average) lead concentration of 35.3 ug/m3 which exceeded the OSHA's AL (Action Level) of 30.0 ug/m3 by a factor of 1.18.
