29 CFR 1926.50(e): Proper equipment for prompt transportation of injured person(s) to a physician or hospital or a communication system for contacting necessary ambulance service, was not provided:      a) An employee who fell 20 feet was allowed to stand and be carried up several flights of a scaffold stairway and then driven to the Hospital in a personal vehicle. Emergency rescue equipment was not available and the local rescue/ambulatory company on Ford Island was not contacted (911) to respond. Injured employee suffered from a concussion, broken/fractured bones in thier elbow and leg, contusions, and deep lacerations.

Guardrailsystems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.      a) Employees were working on a platform next to a parapet wall that was less than 21 inches in height, which was part of the concrete form and was being used as their guardrail system.
