29 CFR 1910.178(m)(5)(i): When powered industrial trucks were left unattended, load engaging means were not fully lowered: Leggett & Platt Commercial Vehicle Products, Inc. - FECAP, located at 377 Woodland Avenue, Elyria, OH: On or about February 28, 2012, the employer did not ensure that Caterpillar (Model C5000-LP, Serial # AT9020835) forklifts load engaging means were fully lowered when the forklift was left unattended in the inventory area of the facility. Employees were exposed to lacerations, fractures, and broken bones hazards if an employee would walk into the forks of the powered industrial vehicle. No additional abatement information is needed for this item.

The descriptions in Column F, case numbers 001, lacked the required information including, but not limited to, the injury or illness and the parts of the body affected. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
