29 CFR 1910.272(k): Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2 1/2 inches:  (a) Grain Drag receiving pit; The employer did not provide adequate guarding of the grain drag receiving pit which had missing and bent protective grating with spaces four inches wide or greater.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  (a) Throughout the grain bin facility; Employees were exposed to electrical hazards due to conduits that were broken, disconnected at junctions or cut off with exposed conductors, such as, but not limited to:  1. Bin # 5; On the east side a broken conduit servicing a lighting fixture, and   2. Bin # 5; On the north side an electrical box had one conduit that was disconnected from the box exposing the live conductor insulation to abrasion and one conduit that was cut off with unguarded exposed dead conductors that could be energized, and   3. Between bins # 2 and 3; A flexible conduit servicing a fan motor was disconnected at the junction with the ridged conduit exposing the conductors to abrasion.
