29 CFR 1926.403(i)(2)(i):     Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means:      a.    On the first floor where employees were installing drywall around electrical panels, the panel cover was removed from an energized electrical panel and not replaced thereby exposing employees to a contact hazard.

29 CFR 1926.501(b)(3):     "Hoist areas." Each employee in a hoist area shall be protected from falling 6 feet (1.8 m) or moreto lower levels by guardrail systems or personal fall arrest systems. If guardrail systems, [or chain, gate, or guardrail] or portions thereof, areremoved to facilitate the hoisting operation (e.g., during landing of materials), and an employee must lean through the access opening or out over theedge of the access opening (to receive or guide equipment and materials, for example), that employee shall be protected from fall hazards by apersonal fall arrest system.      a.     On the southwest side of the roof where the employer set up a material chute to dispose of the roofing materials, the employer did not install a guardrail system or a personal fall arrest system for the area where the employees were entering the chute area thereby exposing the employees to a 30 foot fall hazard.

29 CFR 1926.502(f)(1)(ii):     When mechanical equipment is being used, the warning line shall be erected not less than 6 feet(1.8 m) from the roof edge which is parallel to the direction of mechanical equipment operation, and not less than 10 feet (3.1 m) from the roof edgewhich is perpendicular to the direction of mechanical equipment operation.      a.     On the north end of the roof where a mechanical scaper was used to remove old roofing, the warning line was installed two feet from the edge of the roof thereby exposing employees to a 30 foot fall hazard..
