29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Machine Shop, on or about 3/2/12: XLO Vertical Mill was not provided with a chuck guard. b) Machine Shop, on or about 3/2/12: Mascote Lathe was not provided with a chuck guard. c) Machine Shop, on or about 3/2/12: Clausing single spindle drill press was not provided with a chuck guard. d) Fiber Drum Area, on or about 3/2/12: Rotating shafts on the fiber drum were not provided with guards. Abatement certification must be submitted for this item.

29 CFR 1910.305(g)(1)(iii): Flexible cords and cables were used for purposes prohibited by subparagraphs (A) through (E) of this paragraph: a) Near Fiber Drum, on or about 3/2/12: An extension cord used to provide power to a tape machine was being used as a substitute for permanent wiring. b) Monocoil Area, on or about 3/2/12: An extension cord providing power to a stuffing machine was being used as a substitute for permanent wiring. c) Oven Area, on or about 3/2/12: An extension cord providing power to a stuffiing machine was being used as a substitute for permanent wiring. d) Polishing Area, on or about 3/2/12: An extension cord providing power to two separate polishing machines was being used as a substitute for permanent wiring. e) Fiber Draw, on or about 3/2/12: An extension cord providing power to a fan was used as a substitute for permanent wiring. Abatement must be submitted for this item.
