29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.      Certified Contractors, conducting work at 26800 Woodmont Drive, Suite 5  Suite 10, Perrysburg, OH: On or about February 28, 2012, the employer did not ensure that the employee was protected from fall hazards while performing roofing work on 5/12 and 4/12 pitch roof of the one story residential building. The employee was exposed to fall hazards of approximately 9 feet from the roof to the ground below.    No additional abatement information is needed for this item.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Certified Contractors, conducting work at 26800 Woodmont Drive, Suite 5  Suite 10, Perrysburg, OH: On or about February 28, 2012, the employer failed to ensure that the employee had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection hazards while performing roofing work on 5/12 and 4/12 pitch roof of the one story residential building. The Employee did not use fall protection and was exposed to fall hazards of approximately 9 feet from the roof to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
