29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: JOB SITE: 944 Lemay Ferry Road, St. Louis, MO 63125, at this work site, at this work site, the employer allowed the workers to climb up onto a Werner Aluminum extension ladder, Model: D1540-2 that was positioned approximately 30 inches away from the electrical drop coming from the pole to the structure and were not protected from an electrical hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: JOB SITE: 944 Lemay Ferry Road, St. Louis, MO 63125, at this work site, workers were observed using a Werner Aluminum, Model D1540-2 40ft extension ladder, which was positioned in the rear of the commercial structure, approximately 16 feet 7 inches and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(12): Ladders did not have nonconductive siderails when they were used where the employee or the ladder could contact exposed energized electrical equipment, except as provided in 1926.951(c)(1) of this part: JOB SITE: 944 Lemay Ferry Road, St. Louis, MO 63125, at this work site, a Werner Aluminum, Model D1540-2 40ft extension ladder, positioned approximately 30 inches along side an electrical drop coming from the pole to the structure and workers were allowed to use the aluminum ladder to climb up and down the roof. The workers were exposed to an electrocusion hazard.
