Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to a struck-by hazard from a balancer mounted on an overhead trolley.       (a)	JBS, USA, LLC 800 North 8th Ave., Greeley, CO.  On February 14, 2012, during the morning shift, the WoodHead Aero-Motive, Overhead Balancer at the DeHorner #1, fell down and struck an employee.  The Secondary Chain, a safety anchorage point, to prevent the WoodHead Balancer from falling, was not attached correctly in accordance with the manufacturers safety instructions to prevent the balancer from falling.  This condition exposed employees to a strike-by-hazard.    Among other methods, one feasible and an acceptable method to correct this hazard is to follow the manufacturers' safety instructions for the correct method of attaching a Secondary Safety Chain on all overhead mounted balancers.  Employees, operators and maintenance personnel should also be instructed in the safe, proper use, inspection and maintenance of the equipment based on the manufacturer's recommendations.
