29 CFR 1904.32(a)(4): The employer did not post an OSHA 300A Form or equivalent by February 1 thru April 30. a) Continental Nursing & Rehabilitation Center, Chicago, IL- At the time of inspection on March 2, 2012, the employer had failed to post the OSHA Form 300A Form, the annual summary of work-related injuries and illnesses for calendar year 2011. The OSHA 300A for 2011 was required to be posted from February 1 through April 30 of year 2012. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
