29 CFR 1926.102(a)(1): Eye and face protective equipment were not provided when machines or operations presented potential eye or face injury: (a) R.C. Utility Corporation at N. Speer Blvd & Champa St., Denver, CO 80204: On January 26, 2012, the exposing employer did not ensure employees were provided eye protection when operating a jack hammer on asphalt. Another employee using a shovel at the same area was not provided eye protection as well. This condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards: (a) R.C. Utility Corporation at N. Speer Blvd & Champa St., Denver, CO 80204: On January 26, 2012, the exposing employer did not ensure employees were protected from struck-by hazards by motor vehicle traffic while loading and unloading the boring machine. The equipment trailer was parked in the closed far right lane on N. Speer Blvd near the corner of Champa St. An advance warning sign was not placed on Champa St. warning west bound traffic of the road work ahead. This condition potentially exposed the boring machine operator to being struck by traffic while loading and unloading the equipment.
