29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement: a) 40 Haggerty Drive, West Orange, NJ. Employee is exposed to approximately 16 foot falls to the ground below from potential scaffold collapse or failure as the East side scaffold pole has as its footing two bricks resting on soil which is not a sound support for this Alumapole scaffold system. Violation observed on or about 02/23/2012. UNIVERSAL SIDING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314557752, CITATION NUMBER 1, ITEM NUMBER 2(a), ISSUED ON 10/14/2010, FINAL ORDER DATE 11/05/2010, WITH RESPECT TO A WORKPLACE LOCATED AT HAGGERTY DRIVE & HIMSL COURT, WEST ORANGE, NJ. To abate this violation, the employer must ensure that its employees when working on scaffolds are protected from falls due to potential scaffold collapse by ensuring that scaffold footings are sound and capable of supporting the scaffold when on soil using for example, 2 plates of 1 foot x 1 foot by 3/4 inch plywood nailed together forming a sill plate and the Mud Sill, that comes with the Alumapole scaffolding, shall be spiked to the sill plate. Pursuant to 29 CFR 1903.19(d), the employer must submit documents describing the steps it is taking to ensure compliance, including a description of how these steps will ensure that its employees working on scaffolds will be protected from scaffold collapse by ensuring sound scaffold footings, within 5 days of the date it receives this citation.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) 40 Haggerty Drive, West Orange, NJ. Employee is exposed to 16 foot falls to the ground below as he does not have proper scaffold access as he is disembarking from the scaffold by sliding down the vertical pole of the Alumapole scaffolding system. Violation observed on or about 02/23/2012. UNIVERSAL SIDING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314557752, CITATION NUMBER 1, ITEM NUMBER 3, ISSUED ON 10/14/2010, FINAL ORDER DATE 11/05/2010, WITH RESPECT TO A WORKPLACE LOCATED AT HAGGERTY DRIVE & HIMSL COURT, WEST ORANGE, NJ. To abate this violation, the employer must ensure that its employees are provided with safe access to and from scaffold platforms when these platforms are more than 2 feet above or below a point of access by using, for example, an extension ladder.
