WORKSITE: The junction box, having many knockouts and used for fourplex outlets connected to a 200 foot long extension cord that was laid across the entrance to the worksite, was not protected from physical damage.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    WORKSITE: The covers of three (3) receptacle outlet boxes, connected to a 200 foot extension cord to provide multiple receptacles for the employees were not properly closed, exposing the internal live wiring.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    WORKSITE: The employees, engaged in removing the existing roof were not protected against falling between 29'-6" to the exterior of the building and 42' to the basment by the use appropriate personal fall arrest and/or guardrail systems.

29 CFR 1926.850(h): When debris is dropped through holes in the floor without the use of chutes, the area onto which the material is dropped shall be completely enclosed with barricades not less than 42 inches high and not less than 6 feet back from the projected edge of the opening above. Signs, warning of the hazard of falling materials, shall be posted at each level. Removal shall not be permitted in this lower area until debris handling ceases above.
