29 CFR 1910.22(c):     "Covers and guardrails." Covers and/or guardrails shall be provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.      At the workplace, there was concrete that was cut out and removed from the floor so that plumbing could be installed in the renovated area.  The employees were exposed to a slip, trip, and fall hazard due to the floor holes not being fully covered.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by location in a room, vault, or similar enclosure that is accessible only to qualified persons.           a)  At the workplace, employees were exposed to accidental contact with an overhead 120 volt live wire in the renovated area of the building.  The room was easily accessible to employees, thereby exposing them to electrical shock hazards.

29 CFR 1910.303(b)(1):     Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following consideration:  Heating effects under all conditions of use.    a)  At the workplace, there was an electrical cord that supplied power to an internet router located in the renovated portion of the building that was had several wraps of cord tied into a knot, thereby exposing the employees to a shock and/or fire hazard.

29 CFR 1910.305(b)(1)(ii):     Unused openings in cabinets, boxes, and fittings shall be effectively closed.    At the workplace, there was an electrical service panel that was missing three circuit breakers.  The electrical service panel openings were not covered to prevent accidental contact with the electrical components within the panel.  Employees were exposed to serious physical injuries by electrical shock and arc flash.

29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    At the workplace, there were exposed wires protruding from a junction box that was without a cover located in the remodeling section next to the convenience store, thereby exposing employees to electrical shock and/or fire hazards.
