29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed:    a) Armory, where the surface of the weapons cleaning bench was contaminated with lead and a housekeeping schedule was not in place to ensure all surfaces were maintained as free as practicable of accumulations of lead.

29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:    a) Locations throughout the hospital grounds, where police officers were not provided with a dependable communications system to summons assistance in the event of emergency, assault upon the officer or life threatening incidents. It is generally recognized that law enforcement is a high hazard profession where officers require a radio system that can provide both incoming and outgoing service with fellow officers. Reasonable and feasible abatement methods may include conducting a site survey to evaluate the current infra-structure, upgrades to the existing radio system by adding repeater stations where communications are not adequate to allow for prompt summoning of assistance, issue updated radios, and establish a dispatch communication system.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:    a)  Armory and Gun Ranges, where employees had not received training pertaining to safe work practices and use of personal protective equipment to prevent exposure to lead during tasks such as, but not limited to, gun cleaning.
