Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:    (a)  On or about February 7, 2012, and times prior to that date, for the employees working from an approximately 12-foot wide forklift basket being suspended by a rough terrain forklift, with a width greater than ten inches past the load bearing tires, while installing roof trusses on a residential building located at 3724 East 52nd Street, Sioux Falls, South Dakota.    (b)   On or about February 7, 2012, and times prior to that date, for the employees working from an approximately 12-foot long forklift basket being suspended by a rough terrain forklift with the platform extending greater than two times the load center distance, while installing roof trusses on a residential building located at 3724 East 52nd Street, Sioux Falls, South Dakota.      Abatement Note:  Among other methods, one feasible and acceptable means of abatement to correct the hazard would be to comply with the manufacturer's recommendations and/or to follow the mandatory requirements set forth in ANSI B56.6 Safety Standards for Rough Terrain Forklift Trucks, Section 8.25.1(b) which states:  "Floor dimensions which shall not exceed two times the load center distance listed on the rough terrain forklift truck name plate, measured parallel to the longitudinal center plane of the truck, nor have a width greater than the overall width of the truck measured across the load bearing tires plus 10 inches (250 mm) on either side.  Minimum space for each person on the platform shall not be less than 18 inches (450 mm) in either direction."
