29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    a)  Bank of Guam, Mangilao- On or about December 13, 2011 and at times prior thereto, an employee was using a roller to paint a wall and was not wearing eye protection.

29 CFR 1910.134(k)(6):   The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:    a) Bank of Guam, Mangilao- On or about December 13, 2011and times prior thereto, employees were wearing N 95 Filtering Facepiece Respirators while painting and installing false ceilings.  The employer did not provide employees with a copy of Appendix D.
