29 CFR 1926.453(b)(2)(iv): Employees working in an aerial lift were not standing firmly on the floor of the basket. a) On or about 02/23/2012 at 260 High Street, Perth Amboy, NJ 08861: Employee was using the mid-rail of the JLG 600 AJ Aerial Lift basket as a work platform to perform roofing work at heights of approximately 25' from the concrete ground below. The employee was not standing firmly on the aerial lift basket as required by the manufacturer's instructions listed in the aerial lift basket.

(a) On or about 02/23/2012 at 260 High Street, Perth Amboy, NJ 08861: Employee was exposed to fall hazards while performing roofing work in a JLG 600AJ Aerial Lift without a fall protectiion system connected to the anchor provided in the aerial lift basket. The employee performing roof flashing work was exposed to falls of approximately 25' to the concrete ground below.

(a) On or about 02/23/2012 at 260 High Street, Perth AMBOY, NJ 08861: Employees engaged in roof work at heights of approximately 33' from the ground below were not provided with a fall protection system. Northeast Roof Maintenance Inc., was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(10) which was contained in OSHA Inspection Number 314678921, Citation #1, Item #1, issued on 10/19/2011 with respect to a work place located at 300 Pompton Road, Wayne, NJ 07470 and became a Final Order on 11/03/2011.
