29 CFR 1910.137(b)(2)(viii):     Electrical protective equipment was not subjected to periodic electrical tests in accordance with Tables I-5 and I-6:    (a) Lighting Technicians used Salisbury rubber insulating gloves, Class 00, Type II gloves rated to 500 volts AC when working on energized circuits that had not been electrically tested within the past 6 months to verify integrity of insulating properties. This exposed Lighting Technicians to an electrical shock hazard.

29 CFR 1910.333(a):     Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:    (a) Lighting Technician(s) did not implement electrical safety-related work practices when performing lighting ballast replacement, exposing them to a 277 Volt AC electrical shock and burn hazard, from direct and indirect electrical contacts. Electrical safety-related work practices to be employed, include, but is not limited to electrical hazard evaluation, testing and verification procedures, lock out of electrical circuits, and use of personal protective equipment.
