29 CFR 1910.23(c)(1):  Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, persons can pass, there is moving machinery, or there is equipment with which falling materials could create a hazard:    Employees in the sand processing area were exposed to falls from heights of about 14 feet where guard rails had been removed and not replaced.

29 CFR 1910.303(b)(2):  Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling:      In the diffusion room, a quad box and a duplex box, designed for permanent installation, were being used as extension cords to power equipment including, but not limited to, lighting and the tube grinder machine.

29 CFR 1910.37(b)(2):  Each exit must be clearly visible and marked by a sign reading "Exit":    Exit signs throughout the facility were improperly  posted on and above doors that did not lead directly to a public way.    Note: "EXIT" means that portion of an exit route that is generally separated from other areas to provide a protected way of travel to the exit discharge, which discharge leads directly to a place of safety outside a building.  An example of an "EXIT" is a 2-hour fire-rated enclosed stairway that leads from the 5th floor of an office building to the outside of the building.                                                      ';

29 CFR 1904.32(b)(3): A company executive must certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete:    A company executive did not certify the OSHA form 300.    Note:  An executive of the company is defined as:   1.  The owner of the company (only if the company is a sole proprietorship or partnership);   2.  An officer of the corporation;   3.  The highest ranking official working at the facility; or,   4.  The supervisor of the highest ranking official working at the company.
