Located at 400 Bridge Street Salem, MA, 01970:    On or about December 14, 2011, an employee working with a Brockie International Accu-cut X-33 cutter machine amputated the right pinky and ring finger to the first knuckle and did not return back to work. The employer did not record this injury on the OSHA 300 log for calendar year 2011.

Location:  Facility  	  1. The Clark forklift, model #ECS25 horn did not work and the seat was worn out.   2. The Clark forklift, model #EC500 55F horn did not work and the ignition key was broken off in the ignition.
