29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    (a) Main Building Roof - on or about March 7, 2012 - no conventional fall arrest was being used on a 6 on 12 pitched roof.  Ground to eave elevation was approximately 19 feet.    (b) Garage Roof - on or about March 7, 2012 - no conventional fall arrest was being used on a 10 on 12 pitched roof deck.  Ground to eave elevation wasa approximately 11 feet.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) Center of Building - on or about March 7, 2012 - the ladder used as access from the first story floor to the second story floor did not extend the requisite three feet above the landing.
