29 CFR 1910.242(a):     Hand and portable powered tools or equipment were not kept in safe condition:    On or about 04/23/12 The wood hand of a sledge hammer was split.  This condition exposed workers to the hazard of being struck by a hammer head in the event the handle broke.  The hammer was located near the base of the stairs to the rig floor.

29 CFR 1910.305(b)(1)(ii):     Unused openings in cabinets, boxes, and fittings shall be effectively closed.    On or about 02/23/12 the cover plate for an elbow fitting for electrical conduit 110 VAC was not in place.  The fitting was located in the upper doghouse.  Exposure of electrical equipment could lead to corrosion and potential for sparking flammable vapors.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that unused opening in electrical enclosures are closed.
