29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) There had not been any periodic or at least annual inspections of the lockout procedures conducted, with certification records completed in accordance with 1910.147(d)(6)(ii).

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: (a) In the Fleet area, there was a metal fluorescent light fixture about 7 feet above the floor over the workbench along the north wall that was not effectively grounded in that the ground pin was broken off of the attachment plug. In addition, the outlet that the light fixture was plugged into had high impedance to ground (in excess of 3 ohms). (b) In the Fleet area, the outlet along the north wall near the workbench that the reel cord for the hand lamp, also had high ground impedance (1.7 ohms). (c) In the warehouse at the Route Loading dock, there was a Voice Pick printer with a metal frame that was not effectively grounded in that the ground pin of the attachment plug was broken off and inside the outlet. (d) On the De-Pal mezzanine, the outlet that the Sweed scrap chopper was plugged into was not effectively grounded in that it had a high impedance path to ground (2.02 ohms). (e) The outlet at the bottom of the stairs to the De-Pal mezzanine was not effectively grounded in that it had a high impedance path to ground (in excess of 3 ohms; and 9.28 ohms the second test).
