29 CFR 1910.22(a)(2):   The floor of every workroom was not maintained in a clean and, so far as possible, a dry condition.   On or about February 17, 2012, employees were sliipping and sliding on the ramps in front of the open pits in bays #1 and #2 where an oily residue has accumulated from employees handling vehicle lubricants. The employer does not maintain the floor by providing material to remove the sippery film, exposing employees to the hazard of slips/trips and falls.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the floor is treated to remove the slippery film of oil.

Covers and/or guardrails were not provided to protect personnel from thehazards of open pits, tanks, vats, ditches, etc.  On or about February 17, 2012, employees changing oil in vehicles are required to walk around unattended open pits in bays #1 and #2 that are not equipped with covers or guardrails, exposing the employees to a fall hazard.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all open pits are covered or guardrails are provided.

29 CFR 1910.132(a):   Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  On or about February 17, 2012, employees are required to perform general maintenance on vehicles that include, but are not limited to changing oil and servicing batteries without the employer providing eye protection, exposing the employees to the hazard of being splashed in the eyes by cleaners and vehicle fluids.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that eye protection is provided and utilized by employees changing oil, utilizing cleaners and performing other general vehicle maintenance.

On or about February 17, 2012, employees were servicing and maintaining vehicles in the facility without the employer conducting a hazard assessment, exposing the employees to hazards such as, but not limited to electrical shock and slips/trips and falls. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a suitable facility is installed for employees to flush their eyes and body during an emergency.

On or about February 17, 2012, employees were performing servicing and maintenance duties in bays #1 and #2 while two of the four fire extinguishers were not fully charged, exposing the employees to the hazard of being engulfed by fires.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all fire extinguishers are fully charged for employees to fight incipient stage fires.
