29 CFR 1910.212(a)(1):   Types of guarding. One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    A.B. Dick 1-Color Press (Serial No. 9007-02225, Model No. 360).  There was no guard to prevent employees from getting their clothing or hands caught in the inrunning nip point(s) between the moving rollers during normal production and while cleaning the rollers.    There is no documentation or certification of abatement required for this item.

Guards were not affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.    Goss Urbanite Press. 29 CFR 1910.303(b)(2):  Installation and use. Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling.    Strapper Machine Area.  An electrical box (480V) with multiple outlets was used on the end of a flexible cord for plugging in the strapper machine daily.  The box had pre-punched knockouts, therefore exposing employees to an electrical shock hazard when they used one hand to stabilize the box while plugging in the machine with the other hand.    There is no abatement documentation or certification required for this item.

29 CFR 1910.178(l)(6):  Certification. The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.    The employer did not certify that all employees authorized to operate powered industrial trucks in the workplace had received training and a performance evaluation at least once every three years. 29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings were not effectively closed.      Inserting Line Area.  An electrical box (240V) for the conveyor motor for the inserting line had two openings created by missing knockouts  The box was covered by a piece of corrugated cardboard taped onto the machine to protect the area from dust and debris.    There is no abatement certification or documentation required for this item.
