29 CFR 1910.269(d)(6)(iii): All energy isolating devices needed to control the energy to the machine or equipment were not physically located and/or operated in such a manner as to isolate the machine or equipment from energy sources: (a) On or about February 29, 2012; employees were performing work on a steam conditioning valve, Unit 7, to remove a diaphragm to repair Unit 6. De-energization of all energy sources, such as but not limited to spring tension, was not performed exposing employees to struck-by hazards. IN THE ALTERNATIVE: 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On or about February 29, 2012; employees were performing work on a steam conditioning valve, Unit 7, to remove a diaphragm to repair Unit 6. De-energization of all energy sources, such as but not limited to spring tension, was not performed exposing employees to struck-by hazards.

29 CFR 1910.269(d)(6)(v): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy was not relieved, disconnected, restrained, or otherwise rendered safe: (a) On or about February 29, 2012; employees were performing work on the Unit 7 steam conditioning valve and failed to relieve the spring tension exposing employees to struck-by hazards. IN THE ALTERNATIVE 29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: (a) On or about February 29, 2012; employees were performing work on the Unit 7 steam conditioning valve and failed to relieve the spring tension exposing employees to struck-by hazards.
