29 CFR 1926.20(b)(2):     The employer did not develop an accident prevention program, that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer.    a) 70 Moorlinch Blvd., Medford, NJ:  Employees were installing asphalt paper and preparing for roof shingles on a steep sloped (8 on 12 pitch) roof, were exposed to fall hazards, and were not provided appropriate fall protection for this site.  The employer had not initiated a program for a competent person to conduct frequent and regular inspections of the jobsite, to identify hazards and/or stop the work in the event workers were placed in a hazardous condition.  Observed on or about 03/09/2012.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    a) 70 Moorlinch Blvd., Medford, NJ:  Employees were installing asphalt paper and preparing for roof shingles on a steep sloped (8 on 12 pitch) roof and were exposed to fall hazards between 12 and 23 feet from the roof to the ground, and were not provided appropriate fall protection for this site.  Observed on or about 03/09/2012.     GAN Contractor, LLC; WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 29 CFR 1926.501(b)(13) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314246505 CITATION NUMBER 01, ITEM NUMBER 01a, ISSUED ON 07/21/11.  FINAL ORDER DATE - 08/15/2011.    NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) 70 Moorlinch Blvd., Medford, NJ:  Employees were installing asphalt paper and preparing for roof shingles on a steep sloped (8 on 12 pitch) roof and were exposed to fall hazards with no fall protection.  The employer did not provide each employee training in the recognition and avoidance of these hazards.  Observed on or about 03/09/2012.
