Employees utilize forklifts to move burial vaults from the production area to the storage building on a routine basis.

Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. a. Hupp Stiverson Company - 4300 West Pike, Zanesville Ohio establishment; as of and prior to March 6, 2012; two extension cords were "daisy chained" to provide power for a light fixture on the mixer. The extension cords each have a makeshift splice that is not properly repaired or otherwise protected. This condition poses an increased risk for electrocution to employees with routine exposure from mixer operator, set up and cleaning duties.
