(a) On or about 1/14/11: The OSHA 300 log did not contain a description of the injury or the object or substance that directly injured the person. The section in column F only stated bruised her top left arm; it did not indicate what occurred to cause the bruise. (b) On or about 11/28/11: The OSHA 300 log did not contain a description of the injury or the object or substance that directly injured the person. The section in column F only stated fractured his knee, it did not indicate which knee or what occurred to cause the fracture. Abatement Note: Column F of the log requires a description of the injury or illness, parts of the body affected, and object/substance that directly injured or made the person ill (e.g. second degree burns on right forearm from acetylene torch). NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
