29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:        Woodbridge Roofing and Siding Corporation was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.451(g)(1)(i) which was contained in OSHA inspection number 109174565, citation number 1, item number 3, issued on 10/20/2009 with respect to a workplace located at 785 Central Ave., New Haven, CT and became a final order on 11/16/2009.    Worksite:  The employee who was working from the ladder jack scaffold at approximately 12 feet 8 inches above the ground was not protected from falling to the ground below by the use of a personal fall arrest system.

29 CFR 1926.454(a):     The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.        Worksite:  The employee who was working from the ladder jack scaffold without fall protection at a height of approximately 12 feet 8 inches above the ground was not trained to recognize the hazards associated with the type of scaffold being used.    Woodbridge Roofing and Siding Corporation was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.454(a)(1) which was contained in OSHA inspection number 109174565, citation number 1, item number 4, issued on 10/20/2009 with respect to a workplace located at 785 Central Ave., New Haven, CT and became a final order on 11/16/2009.

Worksite:  The employee who was working on the 10 in 12 pitch roof with an eave height of approximately 14 feet 11 inches above the ground was not protected from fall hazards by the use of guardrails, safety nets or a personal fall arrest system.
