29 CFR 1910.178(l)(1)(i):     The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    Employees engaged in forklift operation activities and employees working in the adjacent areas were exposed to struck-by hazards in that forklift operators were not trained prior to operating the Clark forklift, serial number 355-2091-2541.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    Employees engaged in fabrication activities using in the main mechanic shop and in the alignment shop using the following pieces of equipment were exposed to struck-by hazards in that machine guarding was not provided:    1.  The bearing press, American Chain and Cable Co., serial number not available, in the main mechanic shop    2.  The drill press, manufacturer and serial number not available, in the alignment shop.

29 CFR 1910.215(b)(9):     Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs(b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)    Employees engaged in truck maintenance activities using the following bench grinders in the main mechanic shop were exposed to struck-by hazards in that a tongue guard was not provided.    1.  Performance bench grinder along the northwest work bench in the main mechanic shop.  2.  Dayton bench grinder at the west side of the main mechanic shop.

Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.    Employees engaged in cleaning activities in the main mechanic shop using compressed air were exposed to struck-by hazards in that the compressed air was not regulated to less than 30 psi.

Employees engaged in painting related activities in the paint storage room were exposed to fire and explosion hazards in that the following unused openings in the electrical cabinet along the West wall were not effectively closed:    1.  Knockouts were missing on the side of the electrical cabinet  2.  An opening due to a missing breaker was not effectively closed.
