29 CFR 1926.20(b)(1):  An accident prevention program had not been initiated by the employer.    At the time of the inspection at 15345 Larsen, Overland Park, Ks., 66221, the employer had not established or implemented a safety and health program related to construction to include but not limited to personal protective equipment, and fall protection.  Employees were exposed to struck by and fall hazards while doing residential roofing work.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented.      At the jobsite located at 15345 Larsen, Overland Park, Ks. 66221 employees were performing residential roofing on a 10 in 12 pitch roof without the use of fall protection.  Employees were exposed to fall hazards of 15.4 ft. eave to ground.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    At the time of the inspection located at 15345 Larsen, Overland Park, Ks. 66221, the employer did not provide fall protection training for employees engaged in residential roofing activities.  Employees were exposed to hazards associated with roofing work on a 10 in 12 pitch roof 15.4 feet eave to ground without the use of fall protection.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and stairways had been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders.    At the time of the inspection located at 15345 Larsen, Overland Park, Ks., 66221, the employer did not provide training for proper use of ladders used by employees engaged in roofing activities.  Employees were exposed to falls from ladders 15.4 feet from eave to ground.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.    At the time of inspection at 15345 Larsen, Overland Park, Ks.,  employees engaged in roofing activities were exposed to fall hazard of 15.4 ft. from roof eave to the ground below in that the side rails on the extension ladder used to access the roof did not extend at least three feet above the roof eave.
