29 CFR 1926.1433(d)(3): Hook and ball assemblies and load blocks were not marked with their rated capacity and weight. On March 7, 2012, at 11191 Monroe Rd., Houston, TX, employees were exposed to potential struck by hazards when using a Grove telescopic boom crane with an auxillary ball, without a rated capacity and weight attached to it.

29 CFR 1926.1433(d)(4)(iii): The latch did not close the throat opening so that it would retain slings or other lifting devices/accessories in the hook when the rigging apparatus is slack. On March 7, 2012, at 11191 Monroe Rd, Houston, TX. employees were exposed to struck by hazards when using a 5/8 inch 22 foot, 9 inch chain lifing device whose hook did not have an operational safety latch.
