Nelson Manufacturing Company, located at 6448 US Route 224 Ottawa, OH; On or about March 13, 2012, the employer did not assure the 8ft synthetic sling used to lift and set down a metal Box Tubing of about 30ft long was taken out of service when torn or cut, exposing employees to struck by and crush hazards.

Nelson Manufacturing Company, located at 6448 US Route 224 Ottawa, OH; On or about March 13, 2012, the employer did not assure employees who operated the Pirahna 250T Press Brake were protected from amputation hazards. The point of operation was not guarded, in that approximately 4 inches was left between the ram and the metal part before bending, allowing for employees to access the hazardous area.

Nelson Manufacturing Company, located at 6448 US Route 224 Ottawa, OH; On or about March 13, 2012, the employer did not assure employees were provided with training on protective equipment for the face, hands, and body to protect against electrical hazards while working on or near energized conductors. Employees are required to change fuses on the 240V jib hoist in Building A and are exposed to electrical shock and/or arc flash hazards when verifying zero energy.

Nelson Manufacturing Company, located at 6448 US Route 224 Ottawa, OH; On or about March 13, 2012, the employer did not assure employees used electrical safety related work practices when accessing the 240V electrical box for the jib hoist in Building A to verify zero energy, in that proper PPE was not worn. Employees were exposed to electrical shock and arc flash hazards from 240V.
