29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances shall be of three-wire type and shall be designed for hard or extra-hard usage. Flexible cords used with temporary and portable lights shall be designed for hard or extra-hard usage.     NOTE: The National Electrical Code, ANSI/NFPA 70, in Article 400, Table 400-4, lists various types of flexible cords, some of which are noted as being designed for hard or extra-hard usage. Examples of these types of flexible cords include hard service cord (types S, ST, SO, STO) and junior hard service cord (types SJ, SJO, SJT, SJTO).        On or about 3/9/2012, Job Site    a) The employee used a power strip to energize his tools.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      On or about 3/9/2012, 1st and 2nd Floors    a) There were two unprotected edges exposing the employees to fall hazards.

On or about 3/9/2012, Garage Entrance    a) The employees traveled between the house and garage area, where there was a break in elevation of 32". They did not have a ramp or stairway in this location.
